Intensive English Program Application Instructions

Please send the following:

Intensive English Program Application Form (Please submit either a signed paper copy by mail or an electronic copy)
Affidavit of Support form to apply for an I-20
Document of Financial guarantee (a bank statement or bank letter, on paper)
Copy of the photo page of your passport (on paper)
Send application, documents, and fee (non-refundable) of $50.00 and a housing deposit of $200.00 (credited to account).
CHECK MUST BE IN U.S. DOLLARS AND DRAWN ON A U.S. BANK.

Applying For a F-1 Student Visa:

You, your family, or your sponsor should complete the Affidavit of Support form.

You must mail us a copy with signature.

Send us a copy of the photo page of your passport that shows your full name.

You must also include a document of financial guarantee (a bank statement) in the name of the person paying for the program that proves that you (or your family or your sponsor) has sufficient funds to support your full program of study and personal expenses at Juniata. The bank statement must be an original copy and it must be current (not more than 6 months old.) We must receive a signed original copy of the Affidavit of support and financial guarantee before we can issue an I-20 form.
