Community Service Transcript Notation
Are you an active participant in volunteer programs at Juniata College? If so, you may be eligible for Community Service Transcript Notation!
If you have completed 120 hours of non-paid, non-credit volunteer work over your college career; you are qualified to have "Exemplary Community Service" noted on your transcript upon graduation from the college.
What you need to do:
- Complete a Co-Curricular Transcript Worksheet* available in the Career
and Community Services Office or online.
With this form you must obtain verification of all service hours completed.
- List your position as "volunteer" in the appropriate sections of the
worksheet. In the "Description" section of the worksheet, note the number
of hours devoted to each service activity. See
a sample worksheet.
- Return the completed worksheet to the Community Service Office for processing.
*You may also document other co-curricular activities on this worksheet. Any activities recorded will appear on a Co-Curricular Transcript in your academic file.
