Dental Insurance
Eligible employees may purchase voluntary dental insurance available through AFLAC. Employees have the option of Standard or Premier coverage. Both plans offer a Dental Wellness Benefit twice per policy year for any covered person, for any one treatment listed per visit. Visits must be separated by 150 days or more. Plans offer an Xray Benefit Payable once per visit, per policy year, per covered person. The Wellness and Xray benefits have no waiting period. Plans also offer additional benefits subject to various waiting periods. Benefits will only be paid for specific ADA codes as listed in the policy when a charge is incurred for the covered dental treatment while coverage is in force. There is an Optional Ortho Rider that can be purchased in conjunction with any level plan which includes an annual $1,200 allowance after a two-year waiting period for a Monthly Rate of $29.50.
