This is a summary of the annual report for the JUNIATA COLLEGE GROUP TOTAL DISABILITY PLAN, #23-1352652, July 1, 1999 through June 30, 2000. The annual report has been filed with the Internal Revenue Service, as required under the Employee Retirement Income Security Act of l974 (ERISA).

The plan has a contract with Continental Casualty Company (CNA) to pay all the claims incurred under the terms of the plan. The total premiums paid for the plan year ending June 30, 2000 were $20,829.63.
